Required Before Appointment
Please review the requirements to become a Caregiver at Trend Home Health Services before submitting your WEB application and scheduling an interview.
Caregiver Application Requirements:
All applicants must meet the mandated state of Florida Home Health Care Worker Requirements:
- Professional Certificate of Completion of 75 hours with CURRICULUM HHA/CNA
- RN, LPN, PT, OT, CNA, Valid License
- Liability Insurance
- Initial (Original Certificates) and C.E.U.’s (New Certificates) for HIV / OSHA, Domestic Violence, Alzheimer’s, HIPPA, Assistance with Self Medication, Medical Errors…
- State Issued I.D. and Social Security Card.
- Proof of Residence / Employment Authorization OR Proof of Citizenship: (Passport, Birth Certificate, or Naturalization)
- Physical Exam (Including TB or Chest X-Ray) performed within the last 6 months from a Medical Doctor detailing that the caregiver is “free of communicable diseases.”
- 2 Business References
- CPR Card
- Statewide Background Screening
- Direct Deposit (Void Check)
- COVID-19 Vaccine Card or Exempt Form
- At least one year of experience required as an HHA, CNA, LPN, RN in Home Health Setting
